Streets to Home – Year One Referral

Please complete the information below for the standard Year One Streets to Home package. The only item to select is bed size. All other furnishings are provided as a standard package, with substitutions based on inventory, access, and safety.

  • One bed (Twin, Full, Queen, or King – first floor only)
  • Box spring and frame if available
  • One nightstand
  • One coffee table or end table, based on availability
  • Two dining chairs or bar stools, based on availability
  • Seating based on household size, access, and inventory
